KB Associates Pte Ltd (KBA) can provide qualified and experienced investigators to any company who requires thorough on-site investigations. KBA’s international safety management and investigation expertise can provide an independent ‘team leader’ or ‘team members’ to be incorporated into the client’s investigation team and operational management systems so as to provide appropriate recommendations.

PURPOSE

KBA provides independent incident investigation expertise which will establish the root cause or causes, of the incident using formalised investigation methodology. Our investigation solution involves: detailed and independent investigation and review; establishment of root causes, underlying causes and immediate causes; detailed and comprehensive reporting and making ‘SMART’* recommendations of corrective measures to prevent similar occurrences taking place in future.

SCOPE OF WORK

KBA’s investigation team specialises in collecting, examining and evaluating information to assist the client to establish the root causes, improve safety and avoid future accidents. KBA’s team members’ investigative capabilities include but are not limited to; accident causation; investigative techniques; interview techniques; knowledgeable of the work processes; procedures; personnel; and industrial relations environment of a particular situation.

TEAM PROFILE

Our team is comprised of personnel who are specialists in their respective areas of expertise;

Darren Brunton- CMIOSH

Darren has 23 year’s experience working in the commercial diving industry; prior to this he had a military career of 12 years as a military diving instructor and engineer. Darren is a Charter Member of Institution of Occupational Safety and Health and Trained Senior Investigator, he has been involved with investigating diving incidents involving equipment failures and diver injury/ fatalities for contractors and government authorities. Darren is an approved trainer for IOSH Accident Investigation training courses.

Brendan Kearns

Brendan has 30 year’s experience working in the commercial diving industry; prior to this he was a vehicle mechanic. A holder of a degree in English Law and Trained Senior Investigator, he has been a team member during the investigation of two diving related fatalities and headed equipment failure and product failure investigations.
 

Melissa Ashwell – CMIOSH

Melissa has 21 years + international experience in the field of Health, Safety and Environment Management. Melissa is a Charter Member of Institution of Occupational Safety and Health and Trained Senior Investigator, Melissa has investigated process safety failures, multiple fatalities and incidents within commercial, industrial and residential companies. Melissa is an approved trainer for IOSH Accident Investigation training courses.
 

Alan (Dusty) Millar

Alan has 30 year’s + experience working in the offshore commercial diving industry; prior to this he was a NZ navy Diving Training Officer. He holds a NEBOSH General Certificate, a Post-Graduate Diploma in Maritime Law and a Lloyds Diploma in Vessel, ISM and Warranty Survey. A Trained Senior Investigator, he has been involved with investigating diving incidents involving equipment failures and diver injury.
